The Tale of My Most Spectacular Save
Dr. Robert Montgomery, Professor of Surgery, The Johns Hopkins Hospital

The problem: Where to put a kidney when there are no blood vessels to support it. Answer: Next to the liver, of course.

That was the novel solution chosen by Dr. Robert Montgomery to save the life of 43-year-old Angela Hightower, who was in renal failure after suffering blockages in nearly all her major arteries.

Sent to The Johns Hopkins Hospital from Buffalo, NY, the single mother of seven briefly lost consciousness at the Buffalo airport, recovered enough for the flight, then jumped in a cab to take her from BWI to Hopkins.

Montgomery, 50, is a professor of surgery and director of Hopkins’s Comprehensive Transplant Center. But he’s better known for his earlier five-way, simultaneous kidney swap that won him a spot in the Guinness World Records book. “My kids think that’s pretty cool,” says Montgomery, who splits his time between a loft in Fells Point and his home in Bethesda, where he lives with his four kids and wife Denyce Graves, an opera singer.

“Because of Angela’s blockages in the veins in her chest and abdomen, her blood was trying to take the back roads to get back to her heart,” explains Montgomery. And that was stressing and enlarging the smaller blood vessels.

“We put a catheter in her only remaining functional vessel, in her liver, to dialyze her [to clean her blood of toxins, usually the job of the kidneys],” says Montgomery. But there were two obstacles to the kidney transplant: “After seven pregnancies, her body had built up so many antibodies, it would have rejected a new kidney.” And there were no functioning veins in her abdomen to support a new kidney.

So Montgomery first used a number of new drugs to get her antibodies down. The kidney came from St. Louis, the last one left after a recent eight-way swap.

“Then we had to find a place to put the kidney where it would survive, so I sewed it to the portal vein in the liver, the only one that was still unclogged, which I’m not sure has been done before.” To make that work, blood vessels had to be extended, using transplanted vessels from a cadaver.

It was “touch and go” for a while, he recalls, and closing the abdomen was tough because the kidney was not in the usual place. Then there were five days on a ventilator. “But much to everyone’s amazement, it worked,” he says. And this year, in fact, it appeared on the cover of the American Journal of Transplantation.

“Angela is back in Buffalo, doing well, and raising her children,” says Montgomery. “I remember at Christmastime, all of her kids called me to say thanks.”

“I get sent crazy, difficult cases from all over the country,” he says, “but even among that group, this stood out as the most spectacular save I have done.” —Ken Iglehart

The Tale Of My Most Surprising Diagnosis
Dr. Neal Naff, St. Joseph and Sinai Hospitals 

So what do you tell the anxious family in the waiting room when their loved one has worms in her brain? If you’re Dr. Neal Naff, you more tactfully use the Latin. But in this case, it was good news.

The 69-year-old woman had arrived at the emergency room at St. Joseph Medical Center in 2008 after suffering a seizure.

The fact that it was her first one prompted doctors to order a CT scan of her head, which came back abnormal, as did an MRI.

“The concern,” says Naff, 45, who holds the distinction of being chief of neurosurgery at both St. Joseph and Sinai Hospitals, “was that she had a malignant brain tumor, most likely because of her age, which would be a devastating diagnosis.”

“We had to operate to confirm a diagnosis, which we did four days later. We were using a sophisticated navigation system to get to the tumor, and the surgery was going well, and I was very much expecting to find a brain tumor. But as soon as I located the ‘tumor,’ I knew right away it was unusual.

“The growth was cystic in nature, with gelatin inside, and when I opened it, you could see little objects in there. I knew this was something I’d only read about in textbooks, a parasitic disease.”

A quick test by pathology confirmed his suspicion: “Yes, this is a worm,” the lab said, known as echinococcosis, or sheepherder’s disease.

“Although gross, maybe, the good news was that it’s very treatable,” says Naff.

After the operation, Naff asked the patient, “Were you ever on a sheep farm?” She said yes: She grew up on a sheep farm in Greece as a child. Naff suspects she contracted it from eating contaminated meat or handling sheep. “Even in Greece, it’s rare,” he says.

“But it was such good news to go out and tell the family. Of course, you don’t want to say  ‘worms,’ but ‘a parasitic disease called echinococcosis.’ But the important thing is we can treat it completely. And they were delighted.”

“I trained seven years in neurosurgery at Hopkins and had never seen it there,” says Naff, who lives in Cedarcroft. “Just based on the rarity of problem, it’s got to be my most unusual case.” —Ken Iglehart

The Tale Of My Most Mysterious Case
Dr. Melissa Sparrow, Greater Baltimore Medical Center 

It was a subtle change in the newborn’s cry that led to a new diagnosis—and saved the life of a baby girl. 

Two-month-old Piper was brought to the Greater Baltimore Medical Center ER in March and admitted after her mother noticed she wasn’t feeding well. Yet there was no hint of trouble in the baby’s vital signs. Perhaps it was her reaction to a vaccination the day before. Or maybe, an ER physician thought, it was sepsis, a severe illness in which bacteria overwhelms the bloodstream.

Dr. Melissa Sparrow, GBMC’s director of pediatrics, came on duty at dawn the next morning.She firmly believes that “a good nurse is like the eyes and ears of a doctor.” And it was, indeed, a sharp nurse, Maryellen Helton, who alerted Sparrow that Piper seemed worse—it was the tone of her cry: “You’d better come in here and hear this,” Helton told her. “And she was right,” Sparrow says. “The baby’s cry had turned to a puny little sound.”

A quick exam by Sparrow revealed that Piper’s pupils were dilated and she had become hypotonic—her tiny muscles and joints felt limp. “With the vital signs being okay, this was completely bizarre,” says Sparrow. But after a CT scan came back normal, Sparrow started to rethink the case: “What could change this kid neurologically and have other vital signs be normal?” she asked herself.

The clue was in the cry; It was caused by weakness in the larynx and respiratory muscles that control the vocal chords, just one of the signs Piper’s muscles were being weakened. Sparrow’s conclusion: infant botulism, a rare condition that attacks the neuron synapses that direct the muscles in the body, including those needed to eat or breathe.

Fearful that her tiny charge might get too weak to breathe before an antidote could be made in a California lab, Sparrow had Piper rushed to the pediatric intensive care unit at

Sinai Hospital, with which GBMC works closely. Piper was successfully stabilized until the antidote arrived. That took until the next day, but seemed like an eternity to Piper’s mother, Sarah Bregel. “I was so happy when it got there and they started giving it to her through her IV,” she says. “I texted everyone in my phone, ‘It’s here!’”

Who does Jazzy the boxer puppy have to thank for his next birthday? In large part, Dr. Scott E. Strome, a cancer surgeon more accustomed to healing humans. And thanks to Strome, his owner is a lucky dog, too.

It was at a Bar Mitzvah that Strome ran into a friend, veterinarian Jonathan Kaufman. Kaufman had a rare case: A cancerous tumor in the left cheek of a puppy. He planned to operate to remove the tumor, but asked Strome, who is chairman of the Department of Otorhinolaryngology/Head and Neck Surgery at the University of Maryland School of Medicine, for his help in the operation.

“I thought about it, and said ‘Sure,’” Strome says. “I have two dogs of my own at home, and the night before the operation, I’m looking them over trying to figure out how to do a maxillectomy [removal of cheek bone] on a dog.”

The next day at Eastern Animal Hospital, Kaufman had the anesthesia going, and Strome took the lead in the tumor removal. (Both had volunteered their time.) “I remember I was kind of amused because I’m waiting to be passed instruments when I ask for them, and Dr. Kaufman says, ‘Sorry, this is a vet’s office. Nobody’s going to hand you anything. You have to get your own scalpel.’

“Anyway, the operation went smoothly, but I was struck by the differences between animal and people hospitals. For a person after such an operation, they’d be on a feeding tube and their vital signs monitored for three to five days in the hospital. So I call up the vet the next day and ask, ‘How’s Jazzy?’

“‘Oh, I sent her home with instructions for a soft diet and no chewy toys,’ Dr. Kaufman replied. I thought that was hilarious,” says Strome.

But it was immediately after the operation that the plot thickened: “Jonathan introduced me to the owners, the Budreskis, and I noticed that on Charles Budreski’s face was what looked like skin cancer. I said, ‘You should really come into the clinic to get it checked out.’”

Budreski, 63, of Southeast Baltimore, came into Strome’s office two weeks later. Strome took his history, realized he was a longtime smoker, then noticed something else: An early lesion in his tonsil.

“I told him I was less concerned about the skin cancer and more worried about the tonsil cancer.

A biopsy confirmed it was cancer, but because it had been caught so early, it was treatable. After 33 radiation treatments over several weeks at UMMC, Budreski was on the mend. “Basically, his dog saved his life,” says Strome.

And Jazzy? “She would have died, but she’s doing great now,” he says. “Her face is a little asymmetrical [because of the tissue and bone removed on the left side], but she’s growing fine. So the patient’s doing well and the dog is doing well.” —Ken Iglehart

The Tale Of My Most Gratifying Outcome

For 14-year-old Matthew Costello, son of WMAR-TV news anchor Jamie Costello, the first sign that something was not right was the double vision. 

A subsequent scan showed a cancerous germinoma tumor lodged in the brain stem near the pituitary gland.

The TV host and his wife Donna, a post-operative care nurse at Union Memorial Hospital, traveled up and down I-95 in search of the right treatment for Matthew—from The Johns Hopkins Hospital and the Children’s National Medical Center in Washington, D.C., to Cornell University Medical College in New York.

But on the advice of a friend, they finally consulted late last year with Dr. Joseph Wiley, who is chairman of Sinai’s Department of Pediatrics and interim medical director of Sinai’s Cancer Institute.

There were a couple of different strategies in treating the cancer, Wiley says, but curative

surgery was not one of them, because of where the tumor was lodged. “Germinoma is sensitive

to radiation therapy, but you need a pretty intensive dose,” he says. “And that intensity can cause significant long-term side effects to growth hormones, because it’s really close to the pituitary gland. And it can injure the brain, causing cognition and learning problems—all not very nice.”

Instead, Wiley chose a treatment relying mostly on chemotherapy, “which can reduce, shrink, or melt the tumor with fewer side effects,” followed by smaller doses of more focused radiation.

Wiley describes Matthew as a “quiet, bright, sophisticated kid” who is into sports, and they bonded on the topic of baseball, which Wiley had played as a kid. He lost his hair during the treatment, of course, but was a pretty good sport about the ordeal, says Wiley. “Having no hair was kind of a flashy new punk look for him—I think he actually got into it.”

Now back in Loyola Blakefield High School full time and resuming a little baseball and golf in the spring, Matthew is on the mend.

“I treat a lot of people with potentially fatal cancers, and they usually don’t die. But I got tremendous gratification from this case,” says Wiley, who lives with his wife, a medical technologist, and college-aged daughter in Phoenix. He was recently awarded a $100,000 grant from the Hyundai car company for a study on the effects of cancer on families. “His parents were phenomenal advocates.”
